This is often because their toxic behaviour isn't challenged in the correct way by members of the group, so it continues and continues. Often it can be little things too so they are not always obvious to be countered. For example a player who is very lazy with movement (and thus technically is cheating) might well get away with it a lot against some less observant or less confident players; whilst only one or two call them out on it. So their bad behaviour manages to persist. Another angle is that when challenged the challenge is presented in a hostile fashion so the problem shifts from one of the persons behaviour to one of conflicting egos very fast. That often results in total shut down of any resolution because its two people fighting egos against each other.
